Daily Tadalafil vs. As Needed: 5 mg vs 10–20 mg Explained

Tadalafil is unusual among erectile-dysfunction medications because it can be prescribed in two very different ways.

You can take a higher dose as needed before anticipated sexual activity.

Or you can take a lower dose once every day, without coordinating the dose with sex.

Both approaches use the same active ingredient.

Both require sexual stimulation.

And both are established treatment options for erectile dysfunction.

So why would someone take tadalafil every day instead of only when they expect to have sex?

And is daily tadalafil actually better?

The answer is less about one regimen being “stronger” and more about how treatment fits into a person’s life.

Current European Association of Urology guidance states that there is no clinically significant overall difference in erectile-function outcomes between continuous once-daily tadalafil and on-demand tadalafil. The American Urological Association similarly notes that daily and on-demand tadalafil appear to provide comparable overall efficacy.

The practical difference is the dosing strategy:

As-needed tadalafil provides a long treatment window after a dose.

Daily tadalafil maintains ongoing tadalafil exposure, so medication timing and sexual activity no longer need to be directly linked.

The Short Answer

For erectile dysfunction, tadalafil can be prescribed in two ways.

Tadalafil as needed

Current FDA-approved Cialis labeling describes these as separate dosing strategies rather than interchangeable dose equivalents.

Neither regimen creates an automatic erection.

Sexual stimulation is still required with both.

What Is Tadalafil?

Tadalafil is a phosphodiesterase type 5 inhibitor, or PDE5 inhibitor.

Cialis® is the best-known brand associated with tadalafil, although generic tadalafil is also available.

During sexual stimulation, nitric oxide contributes to production of cyclic guanosine monophosphate, or cGMP, within erectile tissue.

cGMP helps relax smooth muscle and support increased blood flow into the penis.

PDE5 breaks down cGMP.

Tadalafil inhibits PDE5, allowing that erectile-response pathway to remain active longer.

Importantly:

Tadalafil supports an erectile response during sexual stimulation. It does not independently create sexual desire or an erection.

Current Cialis patient information specifically states that sexual stimulation is needed for an erection and that Cialis does not increase male sexual desire.

What Does “As-Needed Tadalafil” Mean?

As-needed tadalafil means taking a dose before anticipated sexual activity rather than taking tadalafil every day.

For most patients, current prescribing information lists 10 mg as the starting as-needed dose, with clinician-directed adjustment to 5 mg or 20 mg depending on efficacy and tolerability.

Tadalafil may be taken before anticipated sexual activity, and clinical studies demonstrated erectile-function benefit for up to 36 hours after dosing.

That long window is one of tadalafil’s defining characteristics.

But “36 hours” is frequently misunderstood.

Does Tadalafil Cause an Erection for 36 Hours?

No.

The 36-hour window does not mean an erection lasts for 36 hours.

It means tadalafil may continue to support the physiological erectile response during sexual stimulation for a prolonged period after a dose.

Sexual stimulation is still required.

An erection lasting longer than four hours is not an expected treatment effect and requires urgent medical attention.

What Does “Daily Tadalafil” Mean?

Current Cialis patient information states that someone taking once-daily tadalafil may attempt sexual activity at any time between doses.

That is why daily tadalafil is often discussed as a treatment strategy that allows less planning around medication.

Why Would Someone Take Tadalafil Every Day?

The main difference is not that daily tadalafil is automatically stronger.

Regular dosing separates medication timing from sexual activity.

Current EAU guidance describes tadalafil 5 mg once daily as an alternative for people who:

  • prefer spontaneous rather than scheduled sexual activity
  • anticipate relatively frequent sexual activity
  • prefer not to link medication dosing directly with sex

The guideline also emphasizes that overall efficacy is not clinically different from on-demand treatment across ED populations.

As-needed tadalafil already has a long treatment window, so it can also provide substantial flexibility.

The distinction is therefore better understood as:

As needed: medication is taken in anticipation of sexual activity.

Daily: tadalafil exposure is maintained continuously, so sexual activity and medication timing are largely separated.

How Long Does Daily Tadalafil Take to Build Up?

This is one of the most frequently misunderstood questions about daily tadalafil.

Tadalafil has a mean terminal half-life of approximately 17.5 hours.

With once-daily administration, tadalafil reaches steady-state plasma concentrations within approximately five days, and steady-state exposure is approximately 1.6 times the exposure following a single corresponding dose.

Those are pharmacokinetic measurements—not a promise about exactly when an individual will experience a clinical effect.

So:

“Daily tadalafil takes five days to start working”

is too simplistic.

The more accurate statement is:

Once-daily tadalafil generally reaches steady-state drug exposure within about five days.

Treatment activity can occur before steady state is reached.

Is Daily Tadalafil Better Than As-Needed Tadalafil?

Not overall.

Current EAU guidance states that there is no clinically significant difference between continuous once-daily tadalafil and on-demand tadalafil across ED populations.

The AUA reaches essentially the same conclusion, stating that on-demand and daily tadalafil appear to produce the same overall level of efficacy, while noting that daily-treatment trials generally used lower doses.

A 2016 pooled analysis by Brock and colleagues compared data from 17 on-demand and four once-daily placebo-controlled tadalafil studies. Both dosing approaches showed efficacy across a range of clinical subgroups, and the analysis did not identify a subgroup that overwhelmingly favored one regimen.

So:

Daily is not inherently better.

As needed is not inherently stronger.

The more useful question is:

Which dosing strategy fits the individual’s ED pattern, sexual frequency, preferences, medical history and response to treatment?

Is 5 mg Daily Tadalafil the Same as 20 mg As Needed?

No.

These are different dosing regimens and should not be compared simply by adding up milligrams.

Daily treatment uses a lower dose repeatedly, creating ongoing tadalafil exposure.

As-needed treatment uses a larger individual dose before anticipated sexual activity.

Current labeling distinguishes the regimens clearly:

Daily ED treatment: 2.5 mg, potentially increased to 5 mg.

As-needed ED treatment: 10 mg starting dose, potentially decreased to 5 mg or increased to 20 mg.

So:

5 mg every day is not simply a weaker version of 20 mg as needed.

And taking several daily-dose tablets should not be treated as a way to recreate an as-needed prescription.

Is 5 mg Daily Tadalafil “Stronger” Because It Builds Up?

Not in the usual sense of the word “stronger.”

Daily tadalafil does accumulate with repeated dosing and reaches steady-state exposure after approximately five days.

But current guidelines do not show that this produces universally better erectile-function outcomes than appropriately prescribed on-demand tadalafil.

Conversely, an as-needed dose contains more tadalafil in a single administration, but that does not make it universally more effective.

Dose size and overall treatment effectiveness are not the same thing.

Does Daily Tadalafil Allow More Spontaneous Sex?

This is probably the clearest practical difference between the two approaches.

With daily tadalafil, medication is already being taken on a routine schedule.

There is no need to decide:

“Should I take my ED medication now because I may have sex later?”

EAU guidance specifically identifies greater separation between medication dosing and sexual activity as one reason some people may prefer daily tadalafil.

However, as-needed tadalafil is already relatively flexible because its treatment window can extend up to 36 hours.

So the difference is not:

spontaneous versus completely scheduled

as much as:

continuous treatment versus medication taken in anticipation of sexual activity.

Does Daily Tadalafil Mean You Can Have an Erection Anytime?

Daily tadalafil allows sexual activity to be attempted at any time between doses.

But it does not produce continuous erections.

Sexual stimulation is still required for tadalafil to support erectile response.

The same is true with as-needed tadalafil.

Daily treatment therefore should not be confused with remaining continuously erect or continuously sexually aroused.

Does Tadalafil Increase Sex Drive?

No.

This one is unusually explicit in the official patient information.

Cialis patient information states that Cialis does not increase a man’s sexual desire.

This matters because erection quality and libido are different aspects of sexual function.

A man can have:

strong desire + difficulty getting or maintaining an erection

or:

low desire + normal erectile ability

or:

both low desire and erectile dysfunction

Tadalafil addresses erectile response. It does not replace an evaluation of persistent low libido.

Can Daily Tadalafil Help if As-Needed Treatment Only Partially Works?

It may be considered in some cases, but this is an individualized treatment decision.

Current EAU guidance discusses tadalafil 5 mg once daily as an option in some men with a partial response to on-demand PDE5-inhibitor treatment.

That does not mean every poor response to as-needed treatment should automatically be converted to daily tadalafil.

  • whether the medication is being used correctly
  • the prescribed dose

Is Daily Tadalafil Better for Frequent Sexual Activity?

Some people who anticipate frequent sexual activity may prefer daily tadalafil because medication dosing is no longer tied to each sexual encounter.

As-needed tadalafil is designed to be used before anticipated sexual activity rather than every day.

For someone who prefers not to take a daily ED medication, that dosing approach may fit better.

Daily tadalafil, by contrast, means ongoing tadalafil exposure throughout treatment.

That distinction can matter when clinicians consider medication interactions and other health factors.

Neither approach is automatically the right choice based only on how often someone has sex.

Are the Side Effects Different With Daily vs. As-Needed Tadalafil?

The active medication is the same, so the adverse-effect profiles overlap.

Commonly reported tadalafil adverse effects include:

  • headache
  • indigestion
  • back pain
  • muscle aches
  • flushing
  • nasal congestion

The dose and exposure pattern differ between daily and as-needed treatment, so an individual’s experience may differ.

The appropriate regimen should therefore reflect both effectiveness and tolerability, not convenience alone.

Can You Take Extra Tadalafil if You Already Take 5 mg Daily?

Do not independently add extra tadalafil to a daily prescription.

Current Cialis patient information instructs people using once-daily treatment not to take Cialis more than once per day.

So:

5 mg daily + an extra 10 or 20 mg before sex should not be treated as a routine do-it-yourself strategy.

Can You Switch Between Daily and As-Needed Tadalafil?

Can Daily Tadalafil Also Be Used for an Enlarged Prostate?

Yes.

Tadalafil also has FDA-approved once-daily uses for symptoms of benign prostatic hyperplasia, or BPH.

Current labeling includes once-daily treatment for:

  • erectile dysfunction
  • signs and symptoms of BPH
  • men with both ED and BPH

For BPH or combined ED/BPH treatment, the labeled dose is 5 mg once daily, subject to individual prescribing considerations.

This is one situation in which daily tadalafil may serve a purpose beyond simply changing the timing of ED treatment.

Does Food Affect Daily or As-Needed Tadalafil?

Tadalafil may be taken with or without food.

Food does not dictate tadalafil dosing in the same way that a high-fat meal can delay sildenafil absorption.

What About Alcohol With Tadalafil?

Alcohol and tadalafil can both contribute to lower blood pressure.

Current prescribing information warns that substantial alcohol consumption—approximately five units or more—can increase the possibility of effects such as dizziness, headache, increased heart rate and lower blood pressure.

Who Should Not Take Tadalafil?

The most important contraindication is nitrate use.

Tadalafil is contraindicated with organic nitrates because the combination can produce a potentially unsafe decrease in blood pressure.

Tadalafil is also contraindicated with guanylate cyclase stimulators such as riociguat.

Other factors may require additional consideration, including:

  • cardiovascular status
  • alpha-blocker treatment

  • kidney impairment
